Our school staff make up part of the multidisciplinary team on the ward and are briefed each day by nursing staff on the medical presentation of our young people during the preceding 24 hours. This includes risk assessment information that allows us to respond to any changes that need to be made on a daily basis. School staff feedback any significant observations or concerns to the ward staff immediately as they arise. Each teacher attends a weekly multidisciplinary team meetings and CPA meetings for their key students.
At the weekly MDT meetings each key tutor feeds back on students’ performance and helps to plan the young person’s next steps. We also hold CPA meetings every six weeks, where the young person reflect on their progress alongside their consultant, key worker, social care team, healthcare, parents/carers and schools and their next steps are planned. A full progress report with tracking data is given to parents/carers and other agencies working with the young person. Contact is made with parents/carers, the relevant local authority and the home school as soon as a student is admitted to our school roll to promote positive relationships.
